L&D · 1:1 active labor

L&D brain sheet — built for the unit you actually work in.

G3P1, GBS positive, on pitocin titrating q30, epidural in at 1430, decels in the last hour. Your sheet should hold the cervical exam log, the FHR trace area, and the pitocin titration timeline on one landscape page — not bury it in a med-surg layout.

What's on it

Every section you actually use in active labor.

Landscape layout because labor moves left-to-right in time. Companion sheets for postpartum (BUBBLE-LE), newborn / mother-baby, and the Mother-Baby Duo bundle.

Active labor sections

  • OB header — G/T/P/A/L, EGA, EDD, blood type, antibody screen, rubella, GBS, HBsAg, HIV
  • Pregnancy summary — risk factors, complications, prenatal care, ultrasound, prior births
  • Membranes — SROM/AROM time, fluid color, ferning, GBS prophy times
  • Cervical exam log — dilation / effacement / station with timestamps
  • FHR + contraction tracing area — category I/II/III, baseline, variability, decels, UA pattern
  • Pitocin titration log — q15-30 rate changes with UA and FHR response
  • Anesthesia — epidural, CSE, time, level, dose, top-ups, pain score
  • Mag protocol — load, maintenance, q1-2h DTR / RR / UOP / mg level
  • Delivery summary — time, type (SVD/operative/C/S), perineum, blood loss, baby Apgars

Postpartum BUBBLE-LE

The companion postpartum sheet (also $3.99) is structured around the standard postpartum assessment mnemonic:

  • Breasts — soft / filling / firm, nipple integrity, feeding plan
  • Uterus — firm/boggy, position, midline/deviated, fundal height
  • Bladder — voided, amount, distended, Foley
  • Bowel — last BM, flatus, sounds, RhoGAM eligibility
  • Lochia — rubra/serosa/alba, amount (scant/sm/mod/lg), clots
  • Episiotomy / laceration — REEDA score
  • Lower extremities — Homans, edema, calf tenderness, SCDs
  • Emotions — bonding, EPDS, baby blues vs. red flags

Who it's for

Labor RNs, OB residents, students on the L&D rotation.

New to L&D

"I'm still learning to read strips."

The FHR section has the category I/II/III prompts so you're forced to characterize the trace every check. The cervical exam log builds Bishop scoring into the layout. You won't forget to ask about GBS or blood type — they're already at the top.

Seasoned L&D RN

"I just want my pit log and exam timeline."

Landscape. Pit titration on the right, cervical log in the middle, FHR area on top. No wasted space on a head-to-toe block you won't fill in. Pair with the postpartum sheet for after delivery.

Mother-baby / couplet

"I take a couplet."

Grab the Mother-Baby Duo bundle ($12, save $8). L&D + postpartum + newborn + couplet sheet covers the full flow from admission to discharge.

Clinical content

The mnemonics and protocols the sheet is built around.

GTPAL & Bishop scoring

GTPAL — Gravida (total pregnancies), Term (≥ 37 wk births), Preterm (20-36 wk), Abortions (< 20 wk losses/terminations), Living children.

Bishop score (cervical favorability for induction) — sum of dilation, effacement, station, consistency, position. Score ≥ 8 generally favorable for vaginal delivery; ≤ 6 typically benefits from cervical ripening.

Both are pre-printed on the OB header so report flows naturally — "G3 P1011 at 39+2, Bishop 8 on admission."

FHR categories (NICHD / ACOG)

  • Category I — normal: baseline 110-160, moderate variability, no late or variable decels, accels OK
  • Category II — indeterminate: most decel patterns, minimal variability, tachycardia — needs intrauterine resuscitation
  • Category III — abnormal: absent variability with late/variable decels or bradycardia, or sinusoidal pattern — requires immediate intervention

The FHR row has check boxes for category I/II/III and a small free-text line for variability, decels, and your intervention (position change, O2, IV fluid bolus, stop pit).

FAQ

Questions L&D nurses ask.

Is the FHR area big enough to actually use?

The FHR section gives you space for baseline, variability, accels, decels, UA pattern, and your characterization (cat I/II/III). It's not a strip itself — the strip lives on the monitor — but it's the area you fill in each time you do a 15- or 30-minute strip review.

Does it have a pitocin titration log?

Yes. The pit titration block is built for q15-30 documentation: time, mU/min, UA pattern, FHR response, and your action. Holds about 12 entries on the landscape layout.

Is there room for a mag protocol?

Yes — there's a small mag panel that fits load + maintenance + your q1-2h reflexes / RR / UOP checks. If you want a full preeclampsia worksheet (BP trend, BUN/Cr/LFTs/uric acid, proteinuria), use the custom builder.

What about C-section recovery?

The L&D sheet covers up through delivery. For PACU recovery from a C/S, use the PACU sheet, then transition to the postpartum BUBBLE-LE sheet on the floor. The Mother-Baby Duo bundle includes everything.

Is there a newborn / couplet version?

Yes — the Newborn / Mother-Baby premade has APGARs, ballard, transition checks, feeds, voids/stools, hep B / Vit K, glucose checks for GDM moms, and a paired mother strip for the couplet.
